Data Collection - Profile Survey

This survey is to be completed by all entities part of the Annual Broadcasting Survey or Annual Telecommunications Surveys.

At the start of each survey, a letter will be sent to participating entities indicating whether they are to file electronically or on paper as applicable.

Entities that are instructed to file electronically must submit their data using the Commission’s web-based Data Collection System (DCS).
Entities that are expected to file on paper will be mailed a paper form.

If you do not receive either of the above requests for information, please get in touch with us for guidance using the On-line form.

Authority

Depending of the form, data may be collected under the authority of the Statistics Act, Revised Statutes of Canada 1985, Chapter S-19, the Telecommunications Act, Section 37 and/or the Broadcasting Information Regulations, 1993.

Completion of this survey is a legal requirement under the Statistics Act, the Telecommunications Act and/or the Broadcasting Information Regulations.

Forms 504 to 507

The following forms have been created:

504 – BITS Licence Surrender
505 – BITS Licence Name Change
506 – Transfer BITS Licence to another Entity
507 – Receipt of BITS Licence from another Entity

Form 802

The Proposed CLEC and CLEC subforms have been replaced by a single subform.

Form 9XX

The ownership (9xx) forms have been updated to facilitate the filing of data and to better reflect the different types of entities that must complete them:

Municipal government entities (including school boards) as well as non-profit corporations, will only need to complete the 912 form (including affidavit) and the 915 form
Cooperatives will need to complete the forms listed above as well as the 914 to provide an account of the percentage of members that are Canadians
Corporations owned entirely by individuals will need to complete forms 912 to 915
Corporations owned by other corporations or by trusts, partnerships or other qualified entities will need to complete forms 912 to 918

In addition, the forms which include data entered in a table (913 to 918) will now be issued with data entered in the previous reporting period. Please ensure that the information is accurate, and make any necessary changes, before re-submitting.

Form REP-U

The form has been modified to capture the reporting entity’s New Media affiliates operating in Canada.
