Telecom Order CRTC 97-694
The Commission received an application dated 20 March 1997 from Voice-Tel Communications (Voice-Tel) for contribution exemptions with respect to single-hop Centrex services and administrative circuits in the (519) 651, 654 and 658 exchanges.

1. Voice-Tel filed an affidavit dated 20 March 1997 with its application.
2. By letter dated 7 April 1997, Bell Canada (Bell) agreed that Voice-Tel's application appears, on a preliminary basis, to qualify for an exemption from contribution.
3. However, Bell noted that Voice-Tel did not have service at the time the affidavit was prepared and submitted that final approval of the requested exemption should be subject to the requirement for a new affidavit to be filed when the services are installed, specifying the services included in the application and affirming the actual arrangements.
4. The Commission notes that Voice-Tel has met the Commission's evidentiary requirements as set out in Applications for Contribution Exemptions, Telecom Decision CRTC 93-2, 1 April 1993.
5. In light of the foregoing, the Commission orders that:
Voice-Tel's application is approved subject to receipt of a sworn affidavit specifying the services included in the application and affirming the actual arrangements, within 30 days of the systems being placed in service. The effective date for both single-hop and administrative circuits is the date of installation.
