Bell TN 7002 ATTACHMENT

1.   Provide a comprehensive description of each of the terms i) scheduled implementation date (SID), ii) engineering receipt date (ERD), and iii) plant test date (PTD) including descriptions of the related major activities accomplished up to each date and their desired outcomes.

2.   For the services identified in Items 107 4(a) and (b) in the proposed tariff pages, provide:

a)   numbers of cancellations in 2005 (1) before the 2 business days, and (2) within 2 business days, of the DD,

b)   typical costs of service installation,

c)   average costs incurred for cancellations in 2005, e.g. engineering, business office activities,

d)   total costs recovered in 2005 through application of Article 20.2 of the company's Terms of Service, and

e)   total number of cancellations included in the study forecast.

3.   For the services identified in Items 107.4 (c) and (g) in the proposed tariff pages, provide:

a)   typical intervals between the SID and the ERD,

b)   typical intervals between the ERD and the PTD,

c)   typical intervals between the PTD and the DD,

d)   number of cancellations in 2005, up to each of the SID, ERD and PTD,

e)   typical costs of service installation,

f)   average costs incurred for cancellations in 2005, e.g. engineering, business office activities, up to and including each of the SID, ERD and PTD,

g)   total costs recovered in 2005 through application of Article 20.2 of the copany's Terms of Service, and

h)   total number of cancellations included in the study forecast.
