These application forms are available in a browser-ready format and may be launched by selecting the form. Depending on the connection speed, the initial install can take up to 10 minutes (at 14.4K) in order to download the necessary files. The install only needs to be done once.
When the forms are completed and submitted electronically, you must file the original hard copy with the Commission at any of our offices. Please note that all confidential submissions must be done in hard copy only or by diskette properly identified.

If you need to fill several copies of the same form, you can make your job easier by filling out any repeat portions, then saving the data and re-using it as many times as you require.
Fill those portions of the form that you plan to re-use.
Position your cursor anywhere on the open form other than on a field (i.e. place it on the text or in the margin).
Click on your RIGHT mouse button
Choose Save Data... to save your input data for later use
The Save As screen will display
Save in choose your preferred location (for example, c:/forms/)
File name for simplicity, choose a name that matches the form (example: CRTC24E)
Save as type accept the default entry presented by the system, i.e. JetForm Data (*.xml)
Click on Save to save the data and to exit the Save As screen.
Launch the matching form into which you will retrieve your saved data. Position your cursor anywhere on the open form other than on a field (i.e. place it on the text or in the margin).
Click on your RIGHT mouse button
Choose Open Data...

Look in enter the location where you previously stored your saved data
The system will display all .xml files listed at that location.
Select the saved data file that matches the form you have just opened.
Click on Open to retrieve your previously saved data into the form.
Caution: as you retrieve the saved data into several copies of the same form, make sure you save each new copy of the form under a distinct name!

Choose Zoom to adjust the magnification level of the open form.
Help text has been provided for those fields that require it, such as for the standardized date field:
Enter the date as follows YYYY-MM-DD.
To view the help message, position your cursor anywhere on the field but do not click your cursor or you will instead activate the field. The help message for that field will display after a three-second delay.
