Re:  Application for costs by PIAC for its participation in Telecom Notice of Consultation 2011-348, Application to Review and Vary Telecom Regulatory Policy 2012-83, and Telecom Notice of Consultation 2012-168 – Our Files 4754-406, 4754-407, and 4754-408

Dear Mr. Lawford:

In reviewing the above-noted applications by PIAC for costs related to its participation in various Commission proceedings, it has come to Commission staff’s attention that the claimants, John Lawford, Michael Janigan, and Jean François Léger, have submitted incomplete Form Is.  

In two instances, Commission staff notes that the forms as filed do not specify the manner in which the claimants report their employment status to the relevant law society, as set out in paragraph 22 of the Guidelines for the Assessment of Costs (the Guidelines).

Commission staff also notes that, in one instance, Mr. Lawford appears to have signed
Form I on behalf of Mr. Janigan. Form I requires the claimant to sign the form, in order to certify that the information contained therein is true and accurate.

Accordingly, Commission staff requests that PIAC file the following information by

Commission staff notes that PIAC was previously sent a letter on 8 June 2012 regarding incomplete applications for costs in connection with four other proceedings. Please note that in the future, applications for costs which are missing the attestation or which are otherwise incomplete will be returned to the applicant.
